Public Health England (PHE) is inviting all women in England over the age of 16 to complete a short survey about reproductive health. It will take about 10 minutes to finish and the information you provide is completely anonymous and confidential. PHE will use this information to develop a plan to improve reproductive healthcare and would like to learn what works well for you and what could be better. 

Reproductive health includes things like pregnancy (childbirth, abortion and problems getting pregnant), contraception, screening (like cervical smears), physical symptoms (like heavy periods), the menopause and sexual enjoyment.

Your participation is voluntary and your answers are completely anonymous and confidential--no one outside the study team will be able to see them. You can stop completing the survey at any time.You will not be contacted by the study team after you submit your answers. 

If you need support with any of the topics mentioned in the survey, sources of information abut reproductive health are provided at the end.

PHE is committed to upholding your rights to privacy and confidentiality.  The information you provide is in strict confidence for the sole purpose of understanding more about your reproductive health needs. Access to your information is controlled and limited to authorised PHE staff.  It will not be disclosed to any third party or used for any other purpose.
